do I need to install a 12 volt flywheel if I have a 6 volt flywheel on a 12 volt Alt.

The flywheel should match the starter

what would happen if I use the same one?,

Different # of teeth.

It is more then teeth.
You would be better off keep the 6V starter till it give up.
I have seen starters modified.
I have seen aftermarket starters with 12v unit but pre-66 teeth and shaft.
Lastly I will search my archives and see if I can find the step-by-step for conversion to 12V on a pre-66 engine/starter. Some machining/grinding is required to relieve the housing also.

"It is more then teeth."
Well that depends on the question. You can use a 6v flywheel and 6 volt starter (see my prior response0 on a 12v system without any issues.
You can't use a 12v flywheel with a 6 volt starter (and vice versa) due to the teeth issue (see my prior response).
If you are looking to convert from a 6 volt system to a 12 volt system than you can use the 6 volt flywheel & starter or you can convert to a 12 volt flywheel and 12 volt starter. If you are already running an IRS you are all set. If you are running a swing than you will likely need to clearance the bell housing and either get a new busing for the starter or run an autostick. You will then obviously need to convert all 6 volt electrical components to 12v.
So the level of complexity is dependent on the question being posed.
